DM3200 no headphone audio

Tascam DM 3200
I am not able to get audio out of the headphone jacks on my DM 3200. I have the Master Fader up and audio is coming out of the Stereo Outputs. I've looked at every setup menu possible. I don't think there is a way that audio could have been redirected away from the headphone jacks. Am I missing something?

UPDATE: I do have headphone audio, it just isn't very loud. I couldn't hear it at all with the control room speakers going. Once I turned them off I could hear the headphone audio, but barely. With the headphone level knob ALL the way up and Pro Tools master fader ALL the way up I am getting a moderate audio level in the headphones but it should be much louder. I'm not using the DIM button. When the control room speakers are on I can make your ears bleed, but there's not much in the cans. Is there some monitor menu item I'm missing?

So how is your master fader? AFAIK, the only thing between faders and CR/headphone levels is the master fader. Even better, tell us your complete signal chain from Protools to monitors/headphones.
 
Ok, I solved my problem, and Arjan P, you are exactly right. It was the Tascam Stereo Fader. In Remote mode I couldn't see that it was down very low. (Unfortunately, the Tascam Stereo Fader doesn't function in Remote mode.) So, I went to another layer, raised the fader and now have lots of level in the headphones.

For what it's worth, I'm coming from Pro Tools into the Tascam Firewire card, routing to tracks 17-18, then out to the Control Room Stereo outputs.
