Model 12 Ch 1/2 USB out duplicated

I'm using my Model 12 with Ableton on a Macbook, and running into an issue I just can't figure out. When I send tracks out to the M12 from Ableton, everying works fine except for channels 1 and 2. I've got the M12 set to the default USB routing, and have panned channels 1/2 hard L/R. Any audio I send to either 1 or 2 ends up on both channels, regardless of their panning in Ableton or which channel I send it to. I've tried sending stereo tracks to both 1/2, individual tracks separately to 1 and 2, all manners of panning. No matter what, I always end up with 1 and 2 as duplicate mono tracks. Any idea what might be going on?
 
I'm on firmware version 1.42. USB audio is set to PC <- MULTI and PC CH1/2 -> CH1/2. I tried 9/10 and MAIN, but got the same result. I've tried it with 2 different computers (both Mac) and found the same behavior. I bought the board second hand, so I'm curious if there are any obscure settings that might be causing this.
 
For anyone else who runs into this issue, I resolved it by reinitializing the board (menu > system > initialize). I never found the root cause.

Happy for you, and thanks for sharing your experience.
Anyway I took the time to test Ableton 11 with my Model 12 and it worked as expected with USB AUDIO options set as yours: each Ableton track is directly sent to one Model 12 mono channel, panning is adjusted on the desk because pan position on Ableton has no influence on mono sends.

Worth mentioning that the Model 12 is easily upset (read: frozen) by the smallest USB glitch and/or temporary samplerate/bitdepth mismatch, even for just a split second. Good quality, shortest length cable is recommended to avoid unexpected crashes.
